Mentor Plus

Mentoring is a one-on-one assignment of an individual to assist a youth through the human development process by providing support, guidance and concrete assistance. The Mentor functions as both a positive role model and as an advocate for the youth in his/her family system. Examples of activities that the mentor can be involved include: recreational & work activities, school related activities such as helping a youth with special assignments when the youth and family believe it is appropriate, social skills and life skills development, peer interpersonal relationship building, personal care, hygiene, exercise, increase awareness of community resources. A mentor should serve as a valuable link in assisting a youth to develop needed skills and relationships.
 
The focus of the Mentor’s interactions and activities are identified on the Referral form and in the Care Plan of the referrent. The goals/needs in the Care Plan should be based on input from the youth, his/her parent/guardian and the Care Coordinator and other members of the Child Team. The time commitment will be dependent on the youth’s needs and program requirements/limitations. Authorized hours are determined by the Care Coordinator.
